AVMED HEALTH PLANS

HAVEN HOSPICE
- Out of the 4,600 hospice programs in the country, Haven Hospice is nationally recognized as one of the leading end-of-life and palliative care programs in the country
- Serves 650 patients daily in 13,000-square-mile, 18-county service area in North Florida
- Has more than 550 employees and nearly 1,000 volunteers
- Recipient of the prestigious Circle of Life Award®, the nation's highest honor in end-of-life and palliative care
SANTAFE SENIOR LIVING
- The Village, Gainesville, Florida, has 341 apartment homes and 66 assisted living apartments, and currently is undergoing a $103 million expansion and repositioning to add 170 apartments and 60 assisted living apartments for a total of 637 dwellings
- Bayview Gardens, a lifecare community, Clearwater, Florida, a bayfront community undergoing a $175 million renovation that includes 138 independent living apartment homes, 18 cottages, 70 refurbished cottages, 56 assisted living apartments, 20 memory support apartments and 40 private skilled nursing suites
- East Ridge Retirement Village, a lifecare community in the town of Cutler Bay, Florida, currently consists of 248 cottages, 51 assisted living apartments and 60 nursing beds. A $120 million redevelopment is under way to replace the Healthcare Center and add 125 apartment homes
- The Terraces at Bonita Springs, Bonita Springs, Florida, is a planned lifecare retirement community with 150 apartment homes, 48 assisted living apartments, 18 memory support apartments, 40 private nursing suites and well-appointed common spaces
